Tolerance in the Transition: Optimizing the Transition to Home Enteral Nutrition

The prevalence of home enteral nutrition (HEN) has tripled over the last 20 years, yet many barriers prevent consistent and effective therapy in the home setting. Challenges include inconsistent definitions of enteral intolerance, lack of standards for assessment and follow-up of HEN patients by national organizations, and inadequate communication within the multidisciplinary care team.

This CE provides a greater understanding of basic clinical assessment skills critical for the management of the home enterally fed patient (specifically during the transition from inpatient to outpatient care), examines common intolerance symptoms and potential root causes, and reviews proper communication channels as a part of the multidisciplinary care team.
